Camper Health & Safety
AF Sports Camps medical personnel are available to assist with camper injuries, illnesses and approved medication needs during camp.
Sports Medicine Guidelines
Please review the following health and medical guidelines before your camper arrives at the United States Air Force Academy.
Medications
Required medication forms must be completed for campers taking prescription, non-prescription or over-the-counter medications during camp.
Concussions
Any camper showing signs or symptoms of a concussion will be removed from participation and evaluated by medical personnel.
Illness
Boarders who leave the dorms because of sickness or infection may not return as a boarder during that camp session.
Camper Medications
Parents must complete the appropriate Sports Medicine medication form for campers who will take or bring medication to camp.
Important Medication Requirements
All medications must arrive in their original, properly labeled container.
Prescription medication labels should include the camper's name, medication name, dosage and frequency.
Parents must complete the appropriate Sports Medicine medication form before camp personnel can administer medication.

If your camper will take or bring medication during camp, complete the appropriate form below and upload the completed document to your camper's ACTIVE account before arrival.
Select the Correct Form
Choose the form that matches the type of medication your camper will bring or receive during Air Force Sports Camps.
Fill Out & Sign
Complete all required camper, parent and medication information. Sign all applicable parent and camper authorization sections.
Save the completed form electronically or scan the signed form so it is ready to upload.
Upload to ACTIVE
Log in to your ACTIVE account and upload the completed medication form to your camper's registration account before arriving at camp.

A camper exhibiting signs or symptoms of a concussion will be removed from participation and will not return to activity during camp.
Immediate Removal
Campers suspected of sustaining a concussion will be removed from practice or competition and evaluated.
Parent Notification
Parents will be contacted after the camper is evaluated and the medical staff determines the appropriate care plan.
Return to Activity
Campers with concussion symptoms will not be permitted to return to activity for the remainder of camp.
